PATIENT CONSENT FOR USE OF RIVERSIDEMYCHART.ORG TERMS AND CONDITIONS

Riverside HealthCare is pleased to offer you information via a web-based version of our computer systems called RiversideMyChart. If you are interested in participating in this service, please read, sign and return this consent form indicating that you have read this Terms & Conditions Statement and agree to it. Riverside HealthCare is a fully integrated healthcare system serving the needs of patients throughout the counties of Kankakee, Iroquois, Will, Grundy and beyond. Riverside HealthCare is comprised of five entities: Riverside Medical Center, Riverside Medical Group, Riverside Senior Life Communities, Riverside HealthCare Foundation, and Oakside Corporation. We think it is important for you to know how we handle information we communicate via the Internet. This Terms & Conditions statement outlines Riverside HealthCare's practices and our sensitivity to your right to privacy. Riverside HealthCare reserves the right to revoke access of RiversideMyChart.org at any time for abusive use of the system.

The use of RiversideMyChart is voluntary and you are not required to use RiversideMyChart or to authorize a RiversideMyChart proxy.

Access to information not included in RiversideMyChart - RiversideMyChart is intended as a secure online source of confidential medical information maintained by Riverside Medical Center and Riverside Medical Group. It contains selected, limited medical information from a patient's medical record and does not reflect the complete contents of the medical record. Copies of medical record information not included on RiversideMyChart may be requested from the Riverside Medical Center's Health Information Management Department. Record duplication fees may apply.

Response to Electronic Communication - Riverside HealthCare will make its best effort to provide a timely response to electronic inquiries. In some cases, delays may occur based on message volume, complexity of messages, availability of clinic staff needed to respond to messages, or technology disruptions. A patient should allow at least two business days for a response. Do not use RiversideMyChart electronic communication for emergency or urgent issues.

EMERGENCY SITUATIONS REQUIRING IMMEDIATE ATTENTION SHOULD NOT BE SUBMITTED ELECTRONICALLY AND THE PATIENT SHOULD CONTACT 911 IMMEDIATELY.

Furthermore, with respect to any electronic communications sent by the patient, Riverside HealthCare is only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, Riverside HealthCare will be unable to provide accurate and reliable services.

Website Links - RiversideMyChart may offer links to related medical websites not managed by Riverside HealthCare. These website link(s) are for a patient's informational purposes only. Riverside HealthCare does not endorse and has not verified the accuracy or security of the information in/on these websites, and the patient should not rely on any of the information found on the websites for purposes of treatment or diagnosis.

E-mail and Text Messaging Privacy - Patients who are users of RiversideMyChart should be aware that they will be notified via e-mail when there is new medical information to be viewed on RiversideMyChart. This means that any person with access to a patient's e-mail will be able to see this notification. This could include the patient's spouse, employer or anyone else that can access a patient's e-mail account. Although no private medical information will be sent in this e-mail, the notification that new medical information is available by accessing RiversideMyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an e-mail address. Please know that if you send us an e-mail communication, it may be shared with the Riverside HealthCare clinical staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on RiversideMyChart will be accessible only to appropriate Riverside HealthCare clinical staff.

Security and Confidentiality - Riverside HealthCare affords the same degree of confidentiality to medical information stored on RiversideMyChart as is given to medical information stored by Riverside HealthCare in any other medium. Riverside HealthCare is committed to protecting the confidentiality of your medical information. We limit Riverside HealthCare employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. Riverside HealthCare shall identify the records released and note the time and date of access each time a patient or his proxy accesses RiversideMyChart. Riverside HealthCare has taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. For other than general information viewing, RiversideMyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Internet Explorer versions 8.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4). You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Riverside HealthCare by viewing the digital certificate. This certificate verifies the connection between the Riverside HealthCare server's public key and the server's identification. User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a RiversideMyChart user, you agree to maintain the security of your medical information by: 1) Changing your password on a regular basis, and 2) Keeping your login ID and password confidential.

Family Access (Proxy) Settings - Riverside HealthCare offers three types of family access (proxy): child, teen and adult.

Child Family Access (Proxy): Child access allows parents to access the electronic medical record of their minor under the age of 12. Access may be requested by the parent or legal guardian (step-parent access requires written consent by both natural parents).

Teen Family Access (Proxy): Teen access allows parents to access the electronic medical record of their minor between the age of 12 and 18. To ensure that the teen retains some privacy, as required by law, the teen patient and parent (proxy) must complete the RiversideMyChart Teen Shared (Proxy) Access consent at a Riverside HealthCare location with signatures witnessed by a Riverside HealthCare employee (step-parent access requires written consent by both natural parents) . The teen has the right to revoke this shared access at any time.

Adult Family Access (Proxy): Adult access allows an adult (18 years of age or older) to access the electronic medical record of another adult. Access may be granted and requested by the patient, legal guardian, or durable power of attorney for healthcare.

Family access (proxy) is discontinued at the time:

  • the patient reaches the age of 18, is otherwise emancipated, or the proxy no longer has legal guardianship of the patient
  • the teen patient revokes parent proxy
  • the patient revokes proxy
  • the proxy no longer has legal guardianship or power of attorney for healthcare for the patient
  • the patient dies

Why won't MyChart let me log in? ›
